Notice of Formation of CREATIVE PLAYGROUND SURFACES, LLC pursuant to Section 206 of the LLCL

(1) The name of the Company is Creative Playground Surfaces, LLC; (2) The date of filing of the articles of organization with the Secretary of State is March 11, 2008; (3) The county within the State of New York in which the office of the Company is to be located is Putnam County; (4) The Secretary of State is designated as the Company’s agent upon whom process against the Company may be served. The address to which the Secretary of State shall mail a copy of any process against the Company served upon the Secretary of State is: Law Office of Camille S. Linson, 13 Parrott Street, Cold Spring, NY 10516; (5) The Company shall not have a registered agent within the State of New York; (6) The Company does not have a specific date of dissolution in addition to the events of dissolution set forth in section seven hundred one of the LLCL; (7) The purpose of the business of the Company is to engage in any and all lawful business activities.
