Notice of Formation of a Domestic Limited Liability
Company ("LLC"). FORK AND GLASS, LLC, with its principal business
location at 10 Division Street, Cold Spring, Putnam County, New York, filed
Articles of Organization with the Secretary of State of New York on April 9,
2008. The Secretary of State of New York has been designated as agent of the
LLC upon whom process against it may be served. The LLC address to which the
Secretary of State of New York shall mail process is 10 Division Street,
Cold Spring, NY 10516. The purpose of the LLC is to engage in any lawful act
or activity for which limited liability companies may be formed.
