entitled to abandoned property in amounts of fifty dollars or more:

A report of unclaimed property has been made to Thomas P.
DiNapoli, the Comptroller of the State of New York, pursuant to Section 701
and/or Section 1316 of the Abandoned Property Law. A list of the names of
the persons appearing from the records of the said insurance company to be
entitled thereto is on file and open to the public inspection at the
principal office of the corporation located at 100 William Street, New York,
NY 10038 where such abandoned property is payable.
Such abandoned property will be paid on or before August
31st next to persons establishing to our satisfaction their right to receive
the same.
On or before the succeeding September 10th, such
unclaimed funds still remaining unclaimed will be paid to Thomas P. DiNapoli,
the Comptroller of the State of New York. Upon such payment this company
shall no longer be liable for the property.
