PHC's Diabetes Self- Management Education Program Awarded American Diabetes Association Recognition
The prestigious American Diabetes Association Education Recognition Certificate for a quality diabetes self-management program was recently awarded to Putnam Hospital Center (PHC). The Carmel hospital's Diabetes Self-Management Education (DSME) Program was recognized as meeting national high quality education standards as an essential component of its diabetes treatment.
To achieve this quality recognition, Putnam Hospital Center voluntarily underwent a rigorous review process by experts in the field of diabetes who evaluated the hospital's program on its ability to provide patients with comprehensive individualized education.
PHC's DSME offers a 10- hour initial training program over a 12-month period for patients with diabetes. One hour of the program involves an assessment of the individual's training needs. The remainder covers instruction in ten content areas contained within the National Standards for Diabetes Self-Management. They are diabetes disease process; nutritional management; physical activity; medications; glucose monitoring; education to prevent, detect and treat acute complications; education to prevent, detect and treat chronic complications through risk reduction; goal setting and problem solving to promote health; psychosocial adjustment; and preconception care, management during pregnancy and gestational management.
